Transport Crew went over the 'Life Flight Ground Nurse Information' form and certain information that needs to be expanded on when it is filled out.  Please  be sure to fill this out for every transport.  

 

Image Trend reporting was also reviewed. There are some fields we need to pay special attention to for transport reports. 

 

Under the following left hand navigation bar choose 'Dispatch Info' >  Please be sure to fill in the 'EMS Response Number' field with the vehicle you responded in 1U511 or 1U512 or any other vehicle you are transporting in.  This field is found directly under the incident number for the call.

 

In the same tab, 'Dispatch Info' > In the 'Complaint Reported By Dispatch' field be sure to choose 'Transfer/Interfacility/Palliative Care' from the dropdown bar. 

 

In the same tab, 'Dispatch Info' > In the 'Type of Service Requested' please choose 'Interfacility Transport' from the dropdown bar. 

 

Now navigate to the 'Unit and Crew' > tab in the left navigation menu.  Please go down to the 'Level of Care of This Unit' and select 'ALS-AEMT' or 'ALS-Paramedic' depending on your crew.  None of the other choices on the list pertains to our EMS agency.
